Appointments

Appointments may be made by phoning the rooms on 9460 9100 during practice hours 9.00am to 5.00pm Monday to Friday.

When attending for a consultation please bring with you the following:

• A referral letter from your general practitioner or other doctor.
• Current x-ray and any relevant older x-rays, MRI or CT scans.
• Pathology results, if appropriate.
• Names of medications you are currently taking.
• Details of any previous surgeries you have had. Please bring a copy of your operation report/s if possible.
• Medicare card or Veterans Affairs card.
• Health insurance details - name of fund, membership number, level of cover.
• Completed Patient Registration Form (PDF)
• Completed Patient Questionnaire Form (PDF)

During your initial appointment your medical history will be recorded and a physical examination will be carried out as part of your clinical assessment. Review of your radiology is an important part of your assessment.
